The following forms are REQUIRED of all student-athletes before engaging in any participation of any kind with DWC athletics programs. Student-athletes will not be allowed to practice or compete with their respective team without all forms reviewed, signed and submitted.

Print ALL attached forms and fill out COMPLETELY. Print clearly. Forms should be brought with you to the department the day of your respective team meetings prior to your respective preseason.
